ATTENTION CITY EMPLOYEES: We have received word that there will be a delay in the mailing of our new Medical ID Cards. All cards should be received by mid-July or earlier. Please continue to use your old card until you receive your new card from CommunityCare.

If you have changed your network/plan or have lost your old ID card, you can register with CommunityCare's new Member Connection website and print a temporary ID card. If you did not previously have insurance through the City of Tulsa, please contact CommunityCare at (918) 594-5242 for your ID/group number prior to registering for Member Connection.
